How to refer
Inclusion Criteria
The Child or Young Person that you wish to refer must be aged between nought and 18 years.
The Child must be registered with and Islington General Practitioner (GP).
Referral Advice for Families and Children and Young People
A Hospital at Home referral can only be completed by a member of the nursing or medical team.
Please speak to the nurse or doctor looking aftr your child about completing a referral.
Referral Procedure for Professionals
A Hospital at Home referral can only be completed by a member of the nursing or medical team that are caring for a child or young person.
If you believe your patient meets the criteria of the Hospital at Home Team then please call the nurse duty on: 07557 202 900 to discuss the referral (if there is no answer, please leave a message and the nurse will get back to you when they are available, it is likely they are on a visit).
For referrals from within Whittington Health, please call the nurse on duty prior to sending a referral to discuss the patient and ensure the service has capacity at that time. Once the child has been accepted, a referral form on ICE needs to be completed and the child will be added to the medical handover so the team can liaise about the child.
For referrals from other NHS Trusts please call the nurse on duty prior to sending a referral to discuss the patient and ensure the service has capacity at that time. Once the child has been accepted, a North Central London referral form needs to be completed and, along with a discharge summary, to be sent to the teams generic email address whh-tr.PaedsHospitalathome@nhs.net
