Health and social care professionals
The district nursing service is available to adults over the age of 16 living in Haringey or Islington who are unable to leave their home to receive care.
We accept referrals from a range of health and social care professionals including carers, hospitals, self-referrals, GPs, practice nurses, care homes, clinical nurse specialists, palliative care, hospices, social workers, dietitians, physiotherapists and occupational therapists.
Referrals should be sent to us at least 48 hours before the patient’s first visit is required. If your patient needs to be seen sooner than this please let us know by phoning our messaging service on 020 7288 3555, otherwise please use the referral form below.
