The hospital at home service works in partnership with acute paediatric services at Whittington and UCLH to provide nursing care in the home for acutely unwell babies, children and young people. The hospital at home team take referrals for all babies, children and young people that fulfil the inclusion criteria and have an Islington based GP.

Client Groups

The conditions that may be relevant for hospital at home include:
  • Gastroenteritis
  • Resolving sepsis (typically discharged from ward)
  • Cellulitis
  • Periorbital cellulitis
  • Pneumonia
  • Infected eczema
  • Feeding: transition to bolus feeds/normal feeding (typically discharged from ward)
  • Babies with jaundice
  • Croup
  • Asthma exacerbation
  • Viral induced wheeze
  • Long-term condition plus viral upper respiratory tract infection (e.g. congenital cardiac condition + URTI where feeding or breathing maybe affected)
  • Bronchiolitis
  • Febrile convulsion
  • Neonate with feeding difficulties
